Tattoo Consent Form

A tattoo consent form is simply agreeing that you as the client give informed consent for me to tattoo you. ✍️(◔◡◔)

Before your tattoo appointment, you must be sober, at least 18 years old, and disclose any potential medical conditions that may affect the tattoo and both of our safety.

Please remember to bring a valid photo ID with you to your appointment to comply with the health department and ensure you are over the age of 18.

You do not have to fill out your consent form prior to the appointment, just the day of! (〜￣▽￣)〜
